Provider Certification

Whether you are an established provider or just getting started we are here for you.

Ohio Department of Developmental Disabilities (DODD) Certification Support

Providers of services to people with developmental disabilities may be self-employed (called independent providers) or agencies. DODD certifies both agency and independent providers of services. Applicants must meet qualifications, pay a non-refundable fee, and include supporting documentation with their application.

An independent provider or agency is prohibited from providing any service until certification is obtained from DODD.

The Ohio Department of Development Disabilities has a lot of resources to help a provider with initial or recertification. Please click the link below for more information.

Fairfield County Board of Developmental Disabilities (Fairfield DD) Certification Support

Fairfield DD can also provide assistance in answering any questions you may have when thinking about becoming a Provider or if you’re renewing your certification. We encourage you to use the free checklists provided below to get you on the right path.
