Annually, providers are required to review their unusual incidents for trends and patterns.
Annual Review
All agency and independent providers shall maintain a log of all unusual incidents. Submit for review quarterly to Fairfield DD by the dates below.
1st Quarter: Jan- March
Due April 15th
2nd Quarter: April-June
Due July 15th
3rd Quarter: July-September
Due October 15th
4th Quarter: Oct.-December
Due January 15th
