Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

Person will require assistance in completing weekly tasks one day a week-right now Thursdays are preferred. This will include going to personal appointments, grocery shopping, meetings, etc. Person will need some help with setting up other appointments, scheduling other transportation, and other tasks as requested. There will be no in home care at this time.
Best ways to communicate with the individual:
It is best to consider tone, calmness and body language when communicating with person. Speak directly to person. Person may not understand initially what is being said. Provide time to process and/or explain information in a different way. Ask person open-ended questions and not facts. When starting to become contrary to what speaker is saying consider person may not understand. Person maybe trying to maintain control over the situation/conversation by deviating from the topic. Continue to ask open ended questions to allow person to control the discussion.

Assistance in home with individualized support to explore his interests, focus on academics, and help teach him positive coping skills when he is upset. He needs assistance with academic planning, completing schoolwork tasks, and transitioning between tasks/activities. (Although he's very bright, he does not like school and experiences a high degree of demand avoidance). He will also need a high level of attunement, help checking in with his "level", and de-escalation assistance when he aggresses. Life skills are also a trigger because of his demand avoidance.
Best ways to communicate with the individual:
Through verbal communication, with additional explanation, hands on examples and physical walkthrough when learning new tasks. He may require patience from those who are communicating with him, especially when he is trying to avoid a task.

Person will develop and follow a structured schedule for home maintenance, grocery shopping, meal planning, cooking, and scheduling appointments/events using accessible tools and supports in order to manage his daily responsibilities. Person will maintain a clean and organized home by following a simple, repeatable schedule. create a simple meal plan, shop for groceries independently, and prepare meals with guided assistance, independently schedule and keep track of appointments and events using a phone and home calendar. Person has Behavior Supports in his plan; both positive BS as well as HRC approved restrictions. Staff will be required to administer medications throughout the shift and keep detailed logs on behaviors and appointments. Person needs assistance with transportation to appointments and to preferred places in the community.
Best ways to communicate with the individual:
Person needs to be heard and feel like they are being listened to at home or in the community. Person wants/needs to be addressed like an adult when being communicated with at home or in the community. Person communicates by speaking. Person can read and write. Person can use a phone and gaming system to communicate with others.

Never leave them alone monitoring the persons level of comfort and engaging in conversation and adapting activities of interests to their abilities and involvement.

Talk in a normal tone of voice. Ask about the persons day and how their day was. Provide encouragement and interaction.
