Scope of Service: HPC 2:1, 6 hours daily; HPC 1:1, 10 hours daily; OSOC 1:1, 8 hours daily.
Needs support with daily hygiene skills, assistance attending medical appointments, assistance with meals, supports with ADLs, assistance with participating in activities of his choice, and supervision.

Individual doesn’t talk much but he does use body language, facial expressions, sounds and a Now and Next book which contains pictures that represent everyday things such as his bed, a cup, the van (for activities in the community), his staff members, the pool at his parents’ house, etc. When the individual wants something, he is prompted to use his book to choose a picture so that those around him understand what he is asking for/about. Supports also use the book to put pictures in on the front so that they can show him what will happen during the day, especially when there is a change in his routine like a Doctor’s appointment, so he understands what to expect. He utilizes a lot of non-verbal cues through use of his body language so supports who know the individual well can tell if he’s jumping up or down or moving his hands in a certain motion, it most likely means that he’s happy- this can usually be confirmed by his facial expressions as well although that isn’t always the case, so supports have to look more into the situation. Finally, the individual uses vocalizations that can show when he’s in happy, upset, anxious or in pain.
