Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

*Medicare Certified Home Health Agency & Bilingual Aide (Spanish/English) Required* Individual wants to improves her health and wellness so that she feels her best every day. Individual wants to explore her interests outside her home so that she can engage in the community and interact with others. Individual is very kind-hearted and a generally happy person. She has a great memory and is a good observer. She also likes to help around the house and enjoys helping others. She loves going to the grocery store and picking out her own snacks. She loves watching YouTube videos (ASMR videos, music videos (Encanto, Selena, and Michael Jackson), eating Cheetos, and playing with small plushies. Staff must understand that when this individual gets upset, she will begin self-injurious behaviors. If someone tries to stop her, she can direct her aggression towards them. On average, she will exhibit this behavior twice a week. She is working to learn coping skills to decrease these emotional outbursts and has Positive Behavior Supports in her OhioISP. Individual needs supervision in her home after she arrives home from her Adult Day Support and on the weekend when her mother works to ensure her health and safety.
Best ways to communicate with the individual:
At home, the individual and her family speak Spanish, but she also speaks/understands English. Her verbal communication is limited and it can be difficult to understand her, at times. She can say around ten words in Spanish and English, such as "Hi," "Bye," "Yes," "No," "Come," and "Okay." She can verbally answer basic Yes/No questions. Often, she will echo the last word or phrase said to her. She can understand clear and specific single-step directions. Because of this, communication regarding schedules, care, activities, etc. will go through her mother, who only speaks Spanish.

She is also looking for transportation to increase access to social and recreational activities. Support is needed to identify community-based opportunities that aligned with her interests, such as attending bowling alleys, dances, and other social events.
