The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

Individual has full assistance to help accomplish her goal of staying comfortable and infection free by following her nursing tasks outlined daily. This is done by nursing staff providing medication administration, including PRN medications, respiratory management, including suctioning and treatments, maintain nutritional support and fluid balance requirements, including GT maintenance and enteral feeding, seizure precautions and control, Skilled Nurse (SN) will provide thermal regulation and proper temperature control, repositioning, ROM exercises, splint a AFOs applications, SN will provide bathing, clothing and diaper changes to maintain cleanliness. All medications must be administered by her nursing provider via G-Tube. Individual has full assistance to address medical related emergencies as well as safety skills related to her medical needs.
Best ways to communicate with the individual:
Individual needs her parents to communicate for her and recognize what she wants. Individual will hold her breath when she is upset or agitated with something she doesn't like. Individual can squeeze someone's hand for yes or to get someone's attention.

If interested, additional rides from work to home could be discussed. The additional rides are on Thursdays and Fridays from around 5:00pm to 5:15pm. Saturdays from around 3:30pm to 3:45pm.

Individual does not use words to communicate. Individual can understand most words being said to them, but doesn't use words to express themself. Individual may use simple sign language like bathroom, stop, no (supports use these with her). Individual uses gestures and behaviors to communicate and will guide supports to something they want; sometimes by reaching for their hand. Provider will communicate with parents/guardians to arrange and manage services.

She needs staff who are prompt, reliable, and in constant communication with changes. Potential for out of town/out of state trips.

Talk to her in person, on the phone, text, email, etc. To speak slowly and calmly are helpful when communicating with her.

To set up initial schedule, communicate any change in schedule, and arrange pick up/drop times via phone call with parent/guardian. When talking with this person provide time to process and time to respond to questions and while in conversation; repeat or rephrase the question if needed and provide additional time to respond.

Take places she wants or likes to go. Assist in all aspects of meal preparation including eating and clean up. Assist with all routine g-tube feedings and cleaning/care of G Tube equipment.
Best ways to communicate with the individual:
Individual communicates with devices, modified sign language or by pointing, vocalizing, etc. Staff can provide choices or use process of elimination to determine what individual wants or needs.
