Person is funny, kind and caring. She needs support with communication and expressing emotions appropriately. This individual may express emotions through yelling, crying, physically, or engaging in OCD routines. Staff would need to support this individual in making daily decisions, promoting independence, and social interactions. Staff would need to understand safety measures while minimizing distractions to help minimize risky behavior. Individual will need assistance if engaged in risky behaviors. Staff will need to assist individual with social interactions, assist with scheduling activities and exploring community activities/ events they may want to do, and transportation to and from. Provide emotional support when the individual struggles with their mental health, especially when experiencing emotional or physical exhaustion. Monitor individual's well-being, and any medical conditions that may affect their mental health or energy levels. Ensure Individual is safe and supported during outings and activities, offering assistance when needed to help engage in community-based events or recreational classes. Staff will need to continue to support individual in developing independent living skills. Individual will need assistance in understanding and managing their finances (with assistance from payee). Staff should help keep track of spending, provide guidance when making purchases, and assist in using funds responsibly. Support staff should be aware of the individual financial understanding limits, by prompting the individual to stay within the planned budget and reminding them of any financial needs or obligations. Keeping receipts and recording purchases will help the individual's payee representative monitor their spending and track funds accurately, report these to payee monthly. Ensure communication is clear with payee about daily needs are being met. Staff will need to help to ensure individual's benefits are maintained, having effective communication with the team about any correspondence from agencies (Metro Housing Auth, JFS, Medicaid, etc.). The individual is actively involved in maintaining their home. Individual will need assistance with completing tasks, as the individual can struggle with details. The individual requires support staff when in the community to help them stay focused and engaged in safety-individual struggles with street safety, parking lot areas, etc. Staff should assist the individual with completing tasks. The individual sometimes experiences behaviors related to OCD or other concerns. Support staff should provide guidance to help manage these behaviors and ensure safety, especially in situations where they may be at risk. Staff should provide direct support and assistance when necessary to help the individual navigate these challenges safely. Staff will need to schedule appointments, assist with transportation to and from medical visits, and follow doctor’s instructions and care plans. The individual will need support with all aspects of mealtime (planning, purchase, prep, cooking, clean up, storing). They will need help with grocery shopping, meal planning, and cooking. The individual needs reminders for daily hygiene, and support staff should ensure the individual follows through with their routine. The individual will need assistance with laundry tasks, ensuring they stay on top of keeping clothes clean and organized.
