If you are looking for opportunities to provide services, be sure to review all RFP’s and respond to any that you meet all listed requirements. Please fill out the form for each posting of interest. Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

Looking for support to select and find new community activities of interest, along with transportation there and back. Individual likes to go to the Y to work out and is open to traveling places outside of town to explore like the Hocking Hills, Burr Oak, putt-putt, etc. They also live independently on their own, so some reminders and support for cleanliness and organization are needed. Individual may need transportation to the grocery store and for errands, when needed. Also, looking for assistance for some healthy meal planning each week.
Best ways to communicate with the individual:
He speaks to communicate and understands what others others say to him. If he does not understand something, it can be explained in a different way. He is very good about speaking up for himself and telling others what he wants.

She needs assistance with completing all household chores, medication administration, and personal hygiene. Supports also need to help her with coordinating and transportation to all community trips. She does have a Behavior Support plan and staff works closely with Behavior Support team. Expected progress towards her outcome includes: assisting her in her journey toward more independence in her daily routine with tasks above, and assistance in transportation in an effort to help her become an integral part of her community
Best ways to communicate with the individual:
She likes to be spoken to in a calm voice. She does not like being told "no" or "stop it" and does best with redirection.
