Keep in mind that your response will be sent to the individual/family so be sure to take this opportunity to make a good impression. The individual/family will choose the providers and will call those they desire to meet for an interview.

What happens next?

All responses go to the SSA/ISC or family, who contacts selected candidates for interviews. Once filled, the RFP is removed and all applicants are notified.

He needs physical assistance to navigate his environment at home and in the community which consists of holding on to the elbow of his caregiver or prompting or use of his cane due to visual impairment. He needs help to participate in ADLs and encouragement to communicate his thoughts and choices.
Best ways to communicate with the individual:
Say his name so he knows you are talking with him and speak clearly. He uses words and body language/behavior to communicate. Give him extra time to process information to formulate a response to questions or when needing to make a decision. He often interacts with people by sharing music with them, and will sing part of a song and wait for someone else to finish the song. He will sing songs based on what mood he is in.

He takes the initiative to pursue things he wants, and he is persistent. He wants to build his decision-making skills while reaching out to his circle of supports when he has questions or wants to talk about ideas. He knows he can improve his decision-making skills by slowing down and thinking through decisions carefully so that he makes decisions with more confidence and commitment. He needs Transportation, some hygiene assistance, assistance grocery shopping, assistance with budgeting, and medication assistance. He is capable of chores around the house including folding and putting away laundry, taking out the trash, light cleaning, and staff can help with more strenuous activities including lifting/carrying laundry baskets, bending, scrubbing, etc. He cooks some of his meals by himself in the microwave, and staff assist him by cooking things on the stovetop and helping him with settings on the oven (since it is difficult for him to see the settings).
Best ways to communicate with the individual:
He enjoys having conversations, whether they are about important topics or just for fun. Face-to-face is great, and he also likes to reach out by phone. Texting works, too, but a phone call is first choice. He has a strong sense of justice and whether something seems right or wrong. He likes to advocate for himself and others. He asks for help verbally whenever he needs it. Communicate with him in a straightforward conversation, respecting him as an adult.

Best ways to communicate with the individual:
This gentleman is deaf. A provider fluent in sign or at least has some working knowledge of sign language is preferred, but not required. Most communication is done via text or phone with his mother, who will always have his most current work and personal schedule. This gentleman is able to communicate via writing things down and reading what others have written. He is able to get his point across with showing you or taking you to what he is wanting you to know or see.
