When something happens that doesn’t match a person’s care or service plan, it should be documented. This is called an unusual incident and is investigated by the provider.
If the incident could harm the person’s health or safety, it is called a
major unusual incident (MUI). An MUI could be alleged, suspected, or actual occurrences of an incident when there is reason to believe the health and welfare of a person may be adversely affected or the person is placed at a likely risk of harm. These are divided into
19 types and have specific investigation processes. Anyone working in the field of developmental disabilities is a mandated reporter and reports should be made immediately or withing 24 hours based on the MUI type.
How to Report
Report by phone at 740.652.7220 Monday - Friday, 8 am to 4:30 pm.
Outside of business hours, call 211 or 740.687.8255 or the DODD Hotline at 1-800-617-6733.
After speaking to to a MUI staff to report, the written report should be submitted to
report@fairfielddd.com by next business day at 3:00 p.m.
All providers shall maintain a log of all unusual incidents and review them each quarter.
1st Quarter: Jan- March
2nd Quarter: April-June
3rd Quarter: July-September
4th Quarter: Oct.-December
Annually, providers are required to review their unusual incidents for trends and patterns. Agency providers should complete this by January 31, and submit them to Fairfield DD by February 28, at
