Eligibility
Eligibility for the Birth to Three Program is established through Help Me Grow. Contact Fairfield County Help Me Grow at 740.681.4881, explain your concerns for your child’s development and ask for a visit by a Help Me Grow Service Coordinator.
To be eligible your child must show a delay in one or more of the following areas:
Problem solving
Social skills
Motor skills
Communication skills
Self help skills
Behavior or emotional skills
Have a medical condition that may affect growth and development
Intake
Based on your request (as identified in your Individualized Family Service Plan) the Help Me Grow Service Coordinator will make a referral to the Birth to Three Program. Once the referral is received and processed, you will receive a phone call to enroll your child and initiate services.
